Clinical notes vs intake documents: what belongs where
CRPO Standard 5.1 scope
CRPO requires an accurate, complete, and legible clinical record for each client, retained for at least 10 years from the last interaction or from the client's 18th birthday, whichever is later. That record includes session notes, treatment plans, and progress documentation — plus the intake and consent documents that belong in the file.
Hivi-X does not replace your EMR or automate amendment tracking, retention schedules, or secure destruction. It helps you collect and organize the intake portion of the record.
EMR vs intake tool
Jane App, Owl Practice, and SimplePractice combine scheduling, billing, and charting. Their forms modules can collect some intake — but pre-session completeness tracking across scattered uploads is often still manual. Hivi-X specializes in checklist-centric intake with item-level approve/reject and exportable packages.
Many practitioners use Jane or Owl for practice management and a dedicated intake tool — or chart on paper and use Hivi-X only for document collection.
Honest Hivi-X positioning
Hivi-X is not a CRPO clinical record system. It does not store session notes, treatment plans, or progress notes as your EMR does. It helps you arrive at session one with consent, insurance, and biopsychosocial questionnaires already on file — then export a PDF package you upload to your clinical record system.
- Intake questionnaires, consent, insurance → Hivi-X checklist + portal.
- Session notes, treatment plans, progress notes → your EMR or paper chart.
- Exportable PDF intake package → upload to Jane, Owl, or secure archive.
