OREA Form 410 document checklist
What OREA Form 410 is
OREA Form 410 is the Ontario Real Estate Association’s standard "Rental Application — Residential" form. Agents use it to capture a prospective tenant’s details for a landlord’s review. It is the application itself — not the proof behind it.
This page is a practical checklist for assembling a Form 410 submission. It is general information, not legal advice. Hivi-X is not affiliated with OREA and does not generate OREA forms; always follow your brokerage’s guidance and current OREA forms.
The documents that go with Form 410
A landlord almost never decides on the application form alone. The supporting documents are what actually get an applicant approved, and gathering them is where most of the delay happens.
- Completed OREA Form 410 (rental application).
- Government-issued photo ID for each applicant.
- Proof of income — pay stubs, employment letter, or Notice of Assessment.
- References — current/previous landlord and sometimes employer.
- Credit-check consent.
- Guarantor information and guarantee documents where required.
Turn it into a reusable Hivi-X checklist
Instead of re-listing these items in an email for every applicant, build the Form 410 package once in Hivi-X and reuse it. Share the checklist with the applicant; they upload each supporting document through the my-id.ca portal; you track what is missing, approve each item, and export a single landlord-ready PDF package. You spend your time on showings, not on chasing the last reference.
