Everything a typical Ontario rehab clinic file needs before the first visit — EHB, WSIB, MVA, consent, and referrals. Verify requirements with insurers, WSIB, colleges, and your privacy officer.

Checklist items

  1. Health history questionnaire (client-centred intake form)
  2. Informed consent for assessment and treatment
  3. Government-issued photo ID
  4. Extended health benefits card (front and back)
  5. Insurer policy / certificate number
  6. MD/NP referral letter (where required)
  7. WSIB claim number, employer, date of injury (WSIB track)
  8. MVA adjuster contact, claim number, accident date (MVA track)
  9. Prior imaging / specialist reports (if applicable)
  10. Hospital discharge summary (post-surgical / inpatient rehab)
  11. Consent for tele-rehab (hybrid/virtual track)
  12. Office manager pre-visit completeness sign-off
