Checklist items
- Health history questionnaire (client-centred intake form)
- Informed consent for assessment and treatment
- Government-issued photo ID
- Extended health benefits card (front and back)
- Insurer policy / certificate number
- MD/NP referral letter (where required)
- WSIB claim number, employer, date of injury (WSIB track)
- MVA adjuster contact, claim number, accident date (MVA track)
- Prior imaging / specialist reports (if applicable)
- Hospital discharge summary (post-surgical / inpatient rehab)
- Consent for tele-rehab (hybrid/virtual track)
- Office manager pre-visit completeness sign-off
