What documents should Ontario realtors collect for a rental application?
An Ontario rental application typically needs five things: government photo ID, proof of income, references, credit-check consent, and the completed application form (often OREA Form 410). Self-employed applicants add a Notice of Assessment, and a guarantor adds guarantee documents. Here is the full checklist:

Identity
- Government-issued photo ID for each applicant (driver's licence, passport, or PR card)
- A second piece of ID where the landlord requests it
Proof of income
- Recent pay stubs (commonly the last 2–3)
- Employment letter stating role, start date, and salary
- Notice of Assessment or T4 for self-employed or commission income
- Bank statements where income is irregular
References & history
- Current and previous landlord references (name and contact)
- Employer or character reference where requested
- Rental history / previous addresses
Consent & forms
- Completed rental application (e.g. OREA Form 410)
