Why a checklist matters

Ontario home care agencies — especially private-pay micro agencies — build intake and hiring documentation from scratch. A checklist gives coordinators and families a shared view of what is required before care starts or before a PSW is assigned.

Client admission items

Core admission documents: demographics and OHIP card, emergency contacts, SDM or POA documentation when relevant, primary physician and specialist contacts, ADL and mobility assessment, current medication list with pharmacy contact, home safety assessment notes, PHIPA consent and information-sharing authorization, signed service agreement or care plan acknowledgment.

Add hospital discharge summary and LTCI or funding authorization when the client situation requires them.

PSW hiring and screening items

Education or PSW certificate verification, criminal record check (CPIC), annual offence declaration, vulnerable sector screening when the role requires it, professional references (minimum two), CPR and First Aid certification, immunization record per agency policy, agency training sign-off log, HSCPOA voluntary registration proof if applicable, and signed employment or contractor agreement.
