Email was not built for therapy intake

Ontario psychotherapists registered with CRPO collect sensitive personal health information before every new client relationship. That can include biopsychosocial history questionnaires, presenting problem and risk screening, informed consent, extended health insurance details, EAP authorization, and telepsychotherapy consent when sessions are virtual.

For many solo and small-group practitioners, much of this still arrives through email and text.

One document is attached to an email. A consent form is still unsigned. A client texts a photo of an insurance card. EAP paperwork sits in a separate thread. Nothing shows whether the file is complete before the 50-minute slot.

The client sees a few simple requests. The therapist sees the full mess — and often burns the first session on admin instead of clinical work.

Why this matters under PHIPA and CRPO

Solo CRPO registrants in private practice are typically health information custodians under PHIPA. Sending psychosocial history and unsigned consent through unencrypted email is a common workaround — not a custody strategy.

CRPO Standard 5.1 expects an accurate, complete clinical record that includes intake and consent documentation. Scattered intake makes it harder to know what belongs in the file before treatment begins.

  • Biopsychosocial history questionnaire

  • Prior mental health treatment history
  • Informed consent for psychotherapy (fees, limits of confidentiality, cancellation)
  • Telepsychotherapy consent (if virtual)
  • Extended health policy details and session limits
  • Insurance card copy (if billing benefits)
  • EAP referral letter and session authorization (if applicable)
