Multi-disciplinary clinic intake standardization

Multi-disciplinary clinic intake standardization

One clinic, three colleges

Ontario multi-disciplinary rehab clinics operate under CPO, CMTO, and CCO simultaneously. Intake forms must respect each profession's consent and record-keeping rules while giving patients one coherent experience.

Standardization model

LayerContents
Shared baseHealth history, consent, ID, insurance, referrals
PT addendumPT-specific history, surgical protocols
RMT addendumCMTO-aligned PHI language, 24h consent documentation note
Chiro addendumCCO consent elements
Payer addendaWSIB, MVA tracks when applicable

Build each layer as a reusable Hivi-X checklist template.

Co-treating workflows

When PT and RMT co-treat an MVA patient, both need the same accident documentation once. Collect on one client record; export packages per provider EMR upload as needed.

General operational information for Ontario physiotherapy and rehabilitation clinics only — not legal, clinical, privacy, or billing advice. Your clinic remains the health information custodian under PHIPA. Hivi-X is document-collection software; it is not an EMR, does not submit WSIB Form 8, and does not certify college or PHIPA compliance.

Answers are general operational information only — not legal, tax, regulatory, clinical, privacy, immigration, or professional advice. Requirements vary by matter, client, and regulator.

Frequently asked questions

Should PT, RMT, and chiro use identical intake forms?
Share a base checklist (health history, consent, insurance) and attach profession-specific items per college expectations — CPO, CMTO, CCO.
How does co-treating affect intake?
One patient may see multiple providers. A shared client record with linked checklists avoids duplicate requests for the same police report or discharge summary.
Does Hivi-X replace each profession's EMR chart?
No. Standardize pre-visit collection; chart separately per provider in your EMR as you do today.

Collect complete patient intake before the appointment

Hivi-X helps Ontario physiotherapy and rehabilitation clinics request health history, consent, insurance cards, referrals, and WSIB/MVA documents with reusable checklists, a secure guest link (no account needed) or my-id.ca portal, self-hosted AI summaries, and exportable packages for Jane, Cliniko, or your EMR.
