What IRCC says the form is for

Additional Family Information (IMM 5406) maps parents, spouse/partner, children, and siblings so officers can verify family composition. Online permanent-residence portals often use a digital version; paper/PDF kits still appear in some workflows. Always use the form edition linked from the current application guide.

IRCC instructions commonly require the form from:

  • the principal applicant
  • the spouse or common-law partner (accompanying or not)
  • each dependent child 18 or over (accompanying or not)

Children under 18 are usually listed on a parent's form rather than completing a separate IMM 5406 — except where a specific kit says otherwise (for example, some dependent-child sponsorship checklists require IMM 5406 for a minor principal applicant, with a parent/guardian signing).

  1. Say why it matters: "IRCC uses this to verify family ties. Incomplete sections slow your file."
  2. Send examples: Highlight where the declaration goes; do not assume everyone has seen this form.
  3. Separate signers: Adult children and spouses get their own form line — not buried in "family docs."
  4. Reject with kindness: "Declaration date missing — upload again when ready." Specific beats scolding.

Per IRCC form instructions for permanent residence, typically: the principal applicant; the spouse or common-law partner (whether accompanying or not); and dependent children aged 18 or over (whether accompanying or not). Confirm current instructions for your application type — some dependent-child sponsorship kits also require IMM 5406 for a minor principal applicant, signed by a parent or guardian.
Is IMM 5406 a custody consent form?
No. IMM 5406 lists family relationships. Non-accompanying-parent consent for minors immigrating is a different form (commonly IMM 5604). Keep both as separate checklist lines when both apply.
Is the declaration section optional?
No. IRCC instructions require answering all questions (use N/A where a section does not apply), then signing and dating the declaration. Missing or inconsistent signatures are a common QC failure.
Can clients upload a photo from their phone?
Yes, if legible. Reject blurry scans on the checklist so they resubmit — do not accept unreadable files because the client is embarrassed.
Should IMM 5406 match other forms exactly?
Names, dates of birth, and relationship details should align across IMM 0008, IMM 5406, and supporting IDs. Flag mismatches before submission. List all children as instructed — including children living with another parent — when the guide requires full disclosure.
