Requirements depend on the stream. For many permanent-residence files involving a minor immigrating without one parent or guardian, IRCC commonly asks for a Declaration from Non-Accompanying Parent/Guardian (IMM 5604), related custody agreements where parents are divorced or separated, or a detailed explanation if consent cannot be obtained. Is IMM 5406 the custody consent form?
No. IMM 5406 is Additional Family Information. Non-accompanying-parent consent for minors immigrating is typically IMM 5604 (hand-signed, often witnessed as the form instructions require), plus photo ID of the signing parent. Do not swap the two on the checklist.
What should RCICs collect besides a court order?
Often: birth certificate linking the child to the parents, passport bio pages, IMM 5604 plus the non-accompanying parent's signed photo ID when required, any custody or separation agreement, death certificate if a parent is deceased, certified translations, and a letter of explanation when a required item cannot be obtained.
