Extended health vs out-of-pocket

Ontario RPs often bill extended health benefits or see clients out-of-pocket with insurer reimbursement. Each path needs different intake documentation:

Extended health / benefits billing:

  • Insurer name and policy or certificate number
  • Insurance card photo (front and back)
  • Session limit or pre-authorization letter (if required)
  • Plan holder name and relationship to client

Out-of-pocket with reimbursement:

  • Fee agreement and receipt preferences
  • Policy details for client-submitted claims
  • Understanding of session caps and eligible provider requirements

Collect these at intake — not when the client asks for a receipt mid-treatment.

EAP is separate paperwork

Employee Assistance Program referrals often require a referral letter, session cap, and employer authorization. Treat EAP as its own checklist track — see EAP referral document workflow.

Manual workflow honesty
