Informed consent document collection

Informed consent document collection for Ontario psychotherapists

Elements of valid informed consent

Informed consent for psychotherapy should be documented and cover the nature of treatment, fees and billing, limits of confidentiality (including mandatory reporting), cancellation policy, and how records are kept. CRPO Professional Practice Standards expect an accurate, complete clinical record that includes consent documentation.

Consent is not a one-time checkbox — updates may be needed when treatment changes, fees change, or modality shifts from in-person to virtual.

Typical elements:

  • Nature and goals of psychotherapy
  • Fees, payment, and cancellation policy
  • Limits of confidentiality and mandatory reporting
  • Record-keeping and client access rights
  • Risks and benefits of treatment

Telepsychotherapy addendum

Virtual sessions require additional consent covering technology platforms, privacy risks of video therapy, emergency procedures, and client location verification. Collect telehealth consent before the first video session — not during it.

Secure collection vs email PDF

Password-protected PDFs sent by email are still email. Health information custodians should implement reasonable safeguards under PHIPA.

Hivi-X lets you add informed consent and telehealth consent as checklist line items. Clients upload signed forms through the my-id.ca portal; you approve or reject each submission before session one.

General information for Ontario psychotherapists and counselling practices only. Not legal, clinical, privacy, or psychotherapy advice. Not affiliated with or endorsed by CRPO or any college. Hivi-X is document-collection software — it is not a licensed psychotherapist, does not provide psychotherapy or clinical assessments, does not certify PHIPA or CRPO compliance, and does not replace your EMR or clinical record obligations. You remain the health information custodian under PHIPA. Consult qualified counsel and your professional liability insurer.

Answers are general operational information only — not legal, tax, regulatory, clinical, privacy, immigration, or professional advice. Requirements vary by matter, client, and regulator.

Frequently asked questions

What must informed consent include?
Typically the nature of treatment, fees and billing, limits of confidentiality, cancellation policy, record-keeping practices, and risks and benefits. Telehealth requires additional technology and privacy elements. Consult current CRPO guidance and your professional liability insurer.
Do I need separate consent for video therapy?
Yes. Telepsychotherapy consent should cover the platform, privacy risks, emergency procedures, and client location. Collect it before the first virtual session alongside standard informed consent for treatment.
Is password-protected email enough for consent collection?
Password-protected PDFs are still email. Custodians should implement reasonable safeguards under PHIPA — a Canadian-hosted portal with encryption and access controls may support your intake workflow alongside professional guidance.
