Clinical notes vs intake documents

Clinical notes vs intake documents: what belongs where

CRPO Standard 5.1 scope

CRPO requires an accurate, complete, and legible clinical record for each client, retained for at least 10 years from the last interaction or from the client's 18th birthday, whichever is later. That record includes session notes, treatment plans, and progress documentation — plus the intake and consent documents that belong in the file.

Hivi-X does not replace your EMR or automate amendment tracking, retention schedules, or secure destruction. It helps you collect and organize the intake portion of the record.

EMR vs intake tool

Jane App, Owl Practice, and SimplePractice combine scheduling, billing, and charting. Their forms modules can collect some intake — but pre-session completeness tracking across scattered uploads is often still manual. Hivi-X specializes in checklist-centric intake with item-level approve/reject and exportable packages.

Many practitioners use Jane or Owl for practice management and a dedicated intake tool — or chart on paper and use Hivi-X only for document collection.

General information for Ontario psychotherapists and counselling practices only. Not legal, clinical, privacy, or psychotherapy advice. Not affiliated with or endorsed by CRPO or any college. Hivi-X is document-collection software — it is not a licensed psychotherapist, does not provide psychotherapy or clinical assessments, does not certify PHIPA or CRPO compliance, and does not replace your EMR or clinical record obligations. You remain the health information custodian under PHIPA. Consult qualified counsel and your professional liability insurer.

Answers are general operational information only — not legal, tax, regulatory, clinical, privacy, immigration, or professional advice. Requirements vary by matter, client, and regulator.

Frequently asked questions

Does Hivi-X replace my session notes?
No. Hivi-X is an intake and document-collection layer. Session notes, treatment plans, and progress notes stay in your EMR or clinical record system.
Can I export intake into Jane?
Yes. Hivi-X generates exportable PDF intake packages you can upload to Jane App, Owl Practice, SimplePractice, or your secure archive.
Does Hivi-X manage 10-year retention and secure destruction?
No. CRPO retention and destruction policies are the custodian's responsibility. Hivi-X helps store electronic intake records but does not set your retention schedule.
