What documents does a physiotherapy clinic need for WSIB intake?
Claim number, employer, date of injury, health history, consent, prior treatment records, and employer or case-manager letters. Form 8 itself is submitted by the clinician through TELUS Health after treatment begins.
Can Hivi-X submit WSIB Form 8?
No. Form 8 must be completed by the treating clinician and submitted within 48 hours via the TELUS Health Provider Portal.
How is WSIB intake different from extended-health intake?
WSIB adds claim metadata and employer correspondence parallel to standard PHI collection. Use a separate reusable checklist so reception does not mix payer requirements.
