What belongs in the clinical record at intake
CRPO Clinical Records Standard 5.1 requires an accurate, complete, and legible record for each client. At intake, that typically includes:
- Documented informed consent
- Telepsychotherapy consent (if applicable)
- Biopsychosocial history and presenting problem documentation
- Risk screening documentation collected at intake
- Insurance or EAP supporting documents relevant to the file
- Referral correspondence (if applicable)
These documents are part of the 10-year retention obligation — from the last client interaction or from the client's 18th birthday, whichever is later.
