Consent complexity in couples work

Couples therapy intake is more complex than individual intake:

  • Individual consent — each partner's rights, fees, and confidentiality limits
  • Joint consent — acknowledgement that conjoint sessions limit individual confidentiality
  • Separate contact details — emergency contacts and communication preferences per partner
  • Relationship history questionnaire — shared presenting problem and individual histories
  • Guardian issues — blended families with minors may need additional documentation

Missing any piece at session one delays clinical work and creates file gaps.

Suggested couples intake checklist items:

  1. Partner A — informed consent (individual)
  2. Partner B — informed consent (individual)
  3. Joint couples therapy consent acknowledgement
  4. Partner A — biopsychosocial history
  5. Partner B — biopsychosocial history
  6. Relationship history / presenting problem questionnaire
  7. Telepsychotherapy consent (if virtual)
  8. Insurance or fee agreement (note: many insurers do not cover couples the same as individual)

Do both partners need to sign consent?
Typically each partner provides individual informed consent, and couples work may also require joint consent acknowledging limits of confidentiality in conjoint sessions. Consult your modality, insurer, and CRPO guidance.
How do I handle a partner who won't complete intake?
Track missing items on the checklist. Document what was requested and received. Clinical decisions about proceeding without complete intake are yours — Hivi-X shows status per item.
Can each partner upload separately through the portal?
Yes. Each partner can use the secure guest link or my-id.ca portal to submit their checklist items. You review and approve per line.
